I hereby apply for my child to attend Life Preparatory School. In completing this application, I understand and agree to comply with and abide by the provisions of the School Student Handbook and the rules and standards set out by the leadership of Life Preparatory School.

The following fees will be invoiced to your student account upon completion of your Re-Enrollment Application for the Fall 2024 Semester.

• Application Fee of $75.00 (Will be invoiced to account)
• Book Fee of $400.00 (Invoiced to Account, Due: July 1, 2024)
• Technology Fee of $100.00 (Invoiced to Account, Due: Due: July 1, 2024)
• First Month Tuition (Invoiced to Account, Due: Due: July 1, 2024)
• Testing Fee of $50.00 (Grades 3rd - 12th only) (Invoiced to Account, Due: Due: July 1, 2024)
• Uniforms are required to be purchased in advance as all students are required to be in full uniform on the first day of school.

*If your child receives a state scholarship through Step Up for Students, our office must receive an updated award ID prior to your child's start date. If it is not received, the following payments above will be required to be paid to secure your child's seat.*
